Registration Procedure

Thank You for choosing to enroll your child here at First United Methodist Preschool.  We look forward to getting to know you and your child.  This paperwork, along with the registration fee will guarantee your place in the fall.

1)    ENROLLMENT FORM:  Please fill in this form as completely as possible.  It is very important that we have emergency phone numbers as well as all possible persons permitted to remove your child.  Please date and sign.

2)   PHYSICAL AND IMMUNIZATION FORMS:  They are available from your pediatrician’s office or the Health Department.

3)   BIRTH CERTIFICATE:  Copy of the original certificate for your child.

4)   NOTARIZED FORM:  Giving us permission to treat your child in an emergency and permission to photograph a school event.
